USING YOUR VISION INSURANCE:

For your convenience, we are pleased to bill vision insurance companies for professional services and materials, given that we are an established provider for that insurance. Our staff will do their best to help you receive your maximum benefits.

All patients, both new and established, must present proof of insurance at the time of service. Please be aware of which primary, secondary and tertiary insurance plans you wish to bill before you schedule your appointment.

All co-payments and exam fees are due in full at the time of service. Any remaining or adjusted balance from insurance payments is the full responsibility of the patient. Vision One will not bill co-payments to Kentucky Medicaid. The patient is responsible for any overages not covered by Kentucky Medicaid.

Vision One is not responsible for benefits quoted by insurance companies as they are only an estimation of benefits.
